ICCPE Warfarin Resource for Pharmacists
Managing Warfarin in the Community - a warfarin interactions poster for use in the dispensaries of community pharmacies.
In 2005, the ICCPE commissioned Caitriona Gowing of the AMNCH, Tallaght, to prepare this poster, which illustrates the many medicines, foods and disease states that may have an impact on INR. Some years ago, Caitriona presented a most successful "Pink, Blue and Brown" course with Dr. Stephen Byrne of UCC's School of Pharmacy, with whom she wrote the course.
It is intended to be printed in A3 format. The poster lists medicines, herbal remedies, foods and disease states that may affect anticoagulation with warfarin. The poster is divided into three sections:
1. Medicines/disease states that may increase INR/anticoagulant effect/risk of bleeding
2. Medicines/disease states that may decrease INR/anticoagulant effect/require higher Warfarin dose
3. Substances/disease states that may either increase or decrease INR
Listings in each of these sections are given in alphabetical order, thus, facilitating user-friendliness. Colour-coding is used for ease of denotation - medicines are in black; herbal remedies in green; foods and tobacco in pink; disease states in blue; and high alert/common interactions are in red.
The warfarin interactions poster is a simple but highly effective idea and it should make life easier for pharmacists and warfarin patients alike.
