This poster presentation documents work undertaken by a Multidisciplinary Team (MDT) which evaluated the reporting of drug allergy information at the Mater Misericordiae Hospital. Following this evaluation, a standardised policy for assessing and documenting drug allergies was produced to reduce errors and improve performance in the future.

This poster outlines research taken to quantify dispensary activity and to assess the impact of changing hospital processes on the pharmacy department.

A large number of patients are treated annually for osteoporosis in the Mater Misericordiae University Hospital (MMUH). As detection strategies improve and more risk factors are identified, the number of patients requiring treatment is increasing. No standardised protocol existed for treatment of these patients in the MMUH. As many new agents became available (Teriparatide, Strontium), it was appropriate that an audit was performed on current practices and a care plan devised for future patients. This poster documents that audit.

Errors in drug prescribing have been found to be the most avoidable medication error, with lack of information about the drug being one of the most frequent causes. This poster summarises research undertaken to identify omissions, if any, in medical undergraduate and postgraduate training with respect to prescribing and medicines management; and to identify the need for expanded pharmaceutical education at medical undergraduate and postgraduate level and the aspects that should be addressed.

In the MMUH ward, stock is provided by an extensive Pharmaceutical Technician and Pharmacy Porter based top-up service, which has been extended over the last number of years to all wards, theatres and some outpatient areas in the hospital. This poster outlines research undertaken to assess and review the current top-up schedule with a view to providing a more effective and efficient service with an improved workflow in the Pharmacy Department.

Vancomycin is a reserve antimicrobial at the Mater Misericordiae University Hospital (MMUH), indicated for the specific treatment of Gram-positive infections resistant or intolerant of standard beta-lactam therapy. Vancomycin therapy requires monitoring of blood levels to minimise the risks of nephrotoxicity and ensure its efficacy. This poster summarises an audit of vancomycin undertaken at the hospital to ascertain the appropriateness of vancomycin TDM in the MMUH with a view to establishing dosing and monitoring guidelines to improve current practice.
